Your first form
Create an account, add a few questions, preview the form and publish it. Ten minutes from sign-up to a link you can send.
Create an account
Go to the sign-up page and enter your name, an email address and a password of at least 8 characters. You can also use Continue with KingsChat instead; see Ways to sign in.
Signing up creates a personal workspace called "Your name's workspace" and puts it on the Free plan. Forms live inside a workspace, and you can add people to it later. A verification email arrives too, but nothing is blocked until you confirm it.
Start a form
On the Forms page, choose New form. Formwork creates an untitled draft and opens the builder. If you would rather start from something ready-made, choose Templates instead (see Dashboard, templates and trash).
Type a title at the top and, if you like, a short description underneath. The description can use bold, italic and links in Markdown style.
Add questions
A new form starts with one empty short-text question. Click into its "Type your question" field and write the question. To use another type, click the type label at the top of the card (it reads Short text) and pick from the list.
To add more, select Add question below the card. A list of question types opens; type a few letters to filter it, use the arrow keys to move, and press Enter to pick one. Then:
- Type the question itself in the field that says "Type your question".
- Switch on Required at the bottom of the card if an answer must be given.
- For choice questions, type each option on its own line. Enter starts a new option, and pasting several lines at once creates one option per line.
Every question needs a title, and a choice question needs at least one option. Otherwise publishing is refused with a message naming the problem, such as "Question 1 has no title."
You never press save. The builder saves about a second after you stop typing, and the top bar shows the state: Saving…, then Saved.
Look at it as a respondent
Choose Preview in the top bar, or use the Preview tab inside the builder. You get the form exactly as respondents will see it, in a desktop or phone frame. Preview runs the real validation, logic and quiz grading, but stores nothing, so you can fill it in as often as you like.
Publish
Choose Publish. A dialog shows the number of questions and offers a Version note so you can remember what this version was for. Choose Publish again.
Formwork now shows the public link, with Copy, Open form and Share options. The link looks like https://your-formwork-address/f/xk3p9d2a; you can change the last part later on the Share tab.
Publishing freezes a copy of the form as version 1. That copy never changes, which is why old responses stay valid when you edit the form later. The next article, Drafts, versions and live settings, explains the details.
See the responses
Open the Responses tab. Until someone submits, it says "Waiting for the first response". After that you get a summary, a chart per question, a table and exports. Reading your responses covers each view.
Note: The Free plan limits the number of live forms, monthly responses and some question types. The limits your workspace has are shown on the Plan page in the sidebar.
