Share a link, QR code or embed

The Share tab

Open Share in the form's top bar. Everyone with access to the form can see it. Changing things on it is limited to workspace owners.

Public link

The public link is https://your-formwork-address/f/<slug>, where the slug starts as eight random characters. Select Copy link to copy it. A badge tells you whether the form is Live, Closed, Archived or Not published, and if it is not live, why.

Custom link

Under Custom link you can replace the random slug with something readable such as spring-survey.

• 3 to 64 characters: lowercase letters, numbers and single hyphens, not starting or ending with a hyphen.
• A few words are reserved, such as admin, api, login and settings.
• While you type, the field checks availability and shows "Available." or the problem.
• Save link applies it.

The old link stops working straight away, which breaks anything you already shared. Change it before you distribute the form, not after. A custom link is a plan feature.

QR code

The QR code points at the public link. Download SVG stays sharp at any size and is the right choice for print. Download PNG is there for tools that need a bitmap; on a server that cannot produce PNG files it says so and points you to the SVG. Both always encode the current link, so download a new one if you change the slug.

Embed

Under Embed, choose a style and copy the code. The preview on the right shows what visitors will get.

Embeds load /embed/<slug>, the same form as the public link, allowed to appear inside other sites. Publish changes and the embedded form updates on its own.

Note: Inside an embed, browsers may block third-party cookies. Forms do not need cookies to be filled in and submitted, but "save and continue later" on the same device and the one-response-per-device rule can stop working in an embed. Use a direct link when you rely on them.

If you want an embed that behaves as if it were part of the page, size it generously. The form does not resize the frame on its own, so a long page needs a taller frame or its own scroll bar.

Adding people and a results link

Tracking where visitors come from

Add utm_source, utm_medium and utm_campaign to the link, and Formwork records them with each response. You can also collect values of your own with hidden fields. Both are described in Calculations, piping and hidden fields.

Search engines

Public forms can be indexed by search engines, but the confirmation, closed, password and embed pages are not. If you do not want a form found, set a password or keep the link private.
