Uploads, signatures, addresses and layout blocks

File uploads with type and size limits, drawn or typed signatures, address fields, and the layout blocks that carry no answer.

File upload

A File upload question lets respondents attach files. In the panel under Uploads:

  • Choose the kinds of file to accept: Images, Documents, Spreadsheets, Archives, Audio and Video. At least one must stay selected.
  • Max size (MB) is the limit per file, from 1 to 50. The default is 10.
  • Max files is how many files can be attached to this question, from 1 to 10.

Each kind maps to specific file extensions.

Formwork checks the file's actual content as well as its name, so renaming a file does not get it past the filter, and names that carry a hidden second extension such as invoice.php.pdf are refused. Uploads start as soon as the respondent picks a file, and a respondent can remove a file before submitting. One visitor can upload at most 300 MB an hour across all their drafts.

Files are stored on the server outside the public web folder. You open them from a response in the Responses tab, and they are served in a way that stops them running as web pages. All uploaded files (ZIP) in the Export menu bundles every file of a form, up to 1 GB at a time. Files are not returned by the REST API; see Forms and responses endpoints.

Uploads count against the file storage of your plan. If the plan has no storage, or the space is used up, respondents see "File uploads are not available for this form" or "This form has run out of file storage". See When you reach a limit.

Signature

A Signature question shows a drawing pad. Respondents draw with a mouse, finger or stylus, with Undo and Clear buttons, or type their name instead. The signature is saved as a PNG image and appears in the response like an uploaded file. It needs JavaScript in the respondent's browser. Like file uploads, a form that uses it can only be published on a plan that includes file storage.

Address

An Address question shows six fields: Street address, Apartment, suite, unit (optional), City, State or region, Postal code and Country. When the question is required, street address, city and country must be filled; the rest are optional. In the table and in exports the answer is one line, with the parts separated by commas.

Layout blocks

Five types display content and never collect an answer. They cannot be required, are not numbered, and do not appear as columns in results.

Images are uploaded in the builder. JPG, PNG, WebP and GIF files up to 5 MB are accepted, then re-encoded on the server, which also removes hidden metadata such as camera location, and resized so that the longest side is at most 2,400 pixels. A GIF is saved as a still PNG. The same rules apply to the cover, logo, welcome and ending images.

Video links are limited to YouTube and Vimeo. YouTube links are rewritten to the youtube-nocookie.com address and Vimeo links to player.vimeo.com. Respondents' browsers load the video from those hosts, so their privacy policies apply to that part of the page.

Headings, text blocks and dividers are available on every plan. Image and video blocks are part of the "all question types" tier.
